CLUB FORMS
Please print and fill out the following forms and turn in to your team manager. The Medical Release needs to be notarized for out-of-state travel (not for Academy players).
CLUB FEES
Academy players: Registration fee $199 + Volunteer hours fee $100* + Membership fee $776 (Total $1075)
Club players: Registration fee $299 + Volunteer hours fee $100* + Membership fee $1596 (Total $1995)
All fees are eligible for a payment plan (6 months) or can be paid in full.
*Volunteer hours fee - Players/family who volunteer 6 hours to the club will get $100 credited to their account.
Uniforms: Approximately $365 and that includes two sets of uniforms, 3rd training kit, jacket, shorts, & socks.
The uniforms are good for two seasons (2026/27 & 2027/28).
